Care Coordination

The purpose of the Care Coordinator project is to create a structure within the school to personally support members of the Ridhwan community and their families in the area of death and dying and for major life events.  Two students from each small group volunteer to be Care Coordinators, responding to needs that are discovered in their group and coordinating care using the Share-the-Care ™ Model and other resources such as Lotsa Helping Hands.

Lotsa Helping Hands website:  http://www.lotsahelpinghands.com/

Lotsa Helping Hands is a good site to coordinate the logistics of care support. This site is very user friendly and helpful. It allows online scheduling for meal delivery, rides to appointments, etc., as well as updates on the care reciever.

Care Coordinator Job Description

Care Coordinators volunteer to be the point persons in their small groups—to be the first contact to determine potential needs and then coordinate care support for students and teachers in the school and their families. A wide-range of care options are available, depending on the unique needs of the individual, and can include making telephone calls to the person on a regular basis to check in, setting up a calendar for volunteers to provide meals and transportation, or organizing a caregiver group to provide support over a longer period of time.

Care Coordinator Expectations

Care Coordinators agree to read the Share the Care ™ and Hard Choices for Loving People books, take part in trainings throughout the year, attend the Bardos workshop, and periodically remind their small groups of the support available through this program, while relying on committee members and fellow Care Coordinators for support.
